Case Manager for Supports Planning Services (full-time)
Requirements: Bachelor’s degree in social work, sociology, psychology, counseling, education, or any related human services field. Must have current driver’s license and reliable vehicle to perform responsibilities required. Must successfully complete criminal background check prior to employment.
Responsibilities include case management services for individuals with disabilities and seniors to achieve their independent living goals through the planning of community-based supports funded by Medicaid. Courteous phone etiquette, excellent written and verbal communications skills, computer skills – literate in Microsoft Word. Preference will be given to experience in providing case management to persons receiving long-term care and/or home and community-based services, working knowledge of Medicaid regulations, and Medicaid waiver.
